Laser Procedure

ALLEGRETTO WAVE™ Eye-Q

Right. From the Start.

Featuring all advantages of the renown ALLEGRETTO WAVE, the ALLEGRETTO
WAVE Eye-Q is a further step towards optimizing the process of laser
vision correction. With customized ablation in mind, all ophthalmic
WaveLight products have been designed as a fully-integrated concept in
order to provide physician and patient the highest level of safety and
reliability in refractive surgery.

The ALLEGRETTO WAVE Eye-Q is the first device ever tha gives the option
of choosing the optimal treatment for the patient from all currently
known methods of refractive surgery.

Q-Value Adjustment

The adjustment of the corneal asphericity (also referred to as the
“Q-Value”) may have influence in affecting the visual acuity under
mesopic and scotopic conditions. The Q-Value is different with every
patient, but probably not optimal for the patient's visual acuity, as
external factors such as wavefront aberrations of the crystalline lens
have a considerable effect on it.

In addition to wavefront-optimized, A-CAT (wavefront-guided) and T-CAT
(topography-guided) treatments, Q-Value Adjustment now adds a 4th
dimension to the treatment of vision disorders by customizing the
asphericity of the cornea. This allows customization and offers a
treatment ideally adjusted to the individual needs of the patient.

Advanced Fine Tuning Capabilities

With the implemented advanced fine tuning options, the physician has the possibility to plan an extended treatment.

The surgeon can adjust the optical zoneas well as the transitional zone
by 0.1mm steps. The refraction input can be leveled in 0.01D. This
allows the user to ideally plan the treatment according to the
individual needs of the patient.

Cross Lines

A cross line projector is integrated into the ALLEGRETTO WAVE Eye-Q.
This device will help physicians to accurately align the patient's eye
for all types of treatments, which allows a more precise correction of
astigmatism and higher order aberrations.

Speed

Efficient treatment with fast pulse repetition minimizes the dehydration
effect of the cornea as well as external influences during operation.
In order to further optimize the refractive treatment itself, the
repetition rate in the ALLEGRETTO WAVE Eye-Q was enhanced to 400 Hz.
This makes the ALLEGRETTO WAVE Eye-Q the fastest excimer laser currently
available on the market. It allows physicians to perform a 6-diopter
myopia correction on a fully- corrected optical zone of 6.5 mm within 15
seconds.

Wavefront-Optimized Ablation Profiles

The ALLEGRETTO WAVE excimer lasers were the first lasers that use
wavefront-optimized ablation profiles, thus integrating an important
wavefront-guided treatment feature in standard laser vision correction.
Laser surgery without a wavefront-optimized ablation profile may cause
alterations of the corneal asphericity in the peripheral correction
area. These so-called induced spherical aberrations in the wavefront
image may cause an alteration of the patient's night or mesopic vision.
A wavefront-optimized ablation profile is designed to prevent such
spherical aberrations of the patient's cornea, whereas the risk of
surgery-related vision problems is minimized.

Small Beam Diameter

Due to the small laser spot with a diameter of 0.95mm (0.68mm FWHM, Full
Width Half Maximum), ALLEGRETTO WAVE laser systems operate with a very
high resolution for even finest corrections of the cornea - as it is
required for customized laser vision correction.

True Optical Zones

The small diameter of the spot allows a high-precision contouring of the
correction profile, while minimizing the total overall ablation depth. A
wavefront-optimized beam profile together with small laser spots allow
operations in a large optical zones and ensuring optimal transitional
zones.

The ALLEGRETTO WAVE Eye-Q is the ultimate laser for refractive surgery.
It has a ceramic laser head with a repetition rate of 400Hz and a
solid-state power supply for long lifetime and energy stability. It also
features a fast video-based "closed loop" eye tracker and a nitrogen
purged beam path.

The user-friendly energy calibration and "closed loop" energy control
integrated in the ALLEGRETTO WAVE Eye-Q significantly increases the
working comfort for the physician and the safety for the patient.

FEMTOLASIK – THE ONLY 100 % LASER OPERATION

The development of refractive surgery has been very fast since 1990.
Major progress has been made in surgical techniques, methods and laser
technology. All this has made laser procedures safer, more comfortable
and the results more precise.
The most recent development in this kind of technology is femtosecond laser.
The new technique enables to make a corneal flap without using
mechanical blade, by creating the flap through photodisruption with a
femtosecond laser.

Laservisioon is the first eye centre in Estonia to use the most recent FEMTO LDV technology in LASIK surgery.
Our new prodecure is called FemtoLasik.
The procedure combines our long experience, knowledge and the World’s leading technology – the Swiss femtosecond laser and superfast Allegretto 400 hz Eye-Q Excimer laser.

OUR FEMTOSECOND LASER

In FemtoLasik procedure, we use high-precision laser made by the
Swiss Ziemer Group. The devices are developed and manufactured in the
same districts where many other types of precision equipment are made,
including the Rolex and Omega watches.

FEMTOLASER PERFORMANCE

The duration of one femtosecond laser pulse is 0,000 000 000 000 001
seconds. The laser sends millions of pulses in a second and apply very
little energy to the eye.

SAFE AND PRECISE OPERATION

  • The eye is operated without using a mechanical blade – the corneal flap is created with a laser.
  • Removing the corneal flap with the laser is more precise and safer than making a cut with a microkeratome.
  • The dimensions of the corneal flap are determined by a top technological computer, ensuring thus its exact right size.
  • Use of laser technology enables to remove the flap faster. This in turn
reduces the duration of the operation and possible risk for
    inflammation and infections.
  • Since no cutting blade contacts the eye, the microtrauma on cellular level will be smaller than by using a microkeratome.
  • Post-surgery studies have shown that use of femtosecond laser does not injure the deeper layers of the cornea.
  • For the patient the use of femtosecond laser means faster recovery,
less discomfort and minimal post-operation prickling sensation.

ALLEGRO Topolyzer

Right. From the Start

The basis for successful treatment of visual disorders is an accurate
diagnosis. Working with the ALLEGRO Topolyzer means using an ideal
device to support the decision-making process for customized laser
vision correction. By determining the patient’s exact visual correction
needs, it enhances the range of possible treatments, from WaveLight's
“wavefront-optimized” procedure to the highly sophisticated
topography-guided treatment.

The ALLEGRO Topolyzer with its corresponding T-CAT software has been
specifically designed to measure and generate high resolution treatment
data for eyes with severe cornea irregularities. It was developed for
the most difficult cases, in which standard wavefront measurement and
treatment would prove ineffective from a medical point of view.

Due to its extraordinary high resolution of the cornea resulting from a
large placido disc with 22,000 measuring points, the ALLEGRO Topolyzer
is an ideal pre-operative tool for laser treatment. In addition to
producing a refractive map to display the influence of aberrations, the
ALLEGRO Topolyzer is fitted with an interactive elevation map, allowing
the individual selection of reference body, radius and asphericity.

ALLEGRO T-CAT Treatment

ALLEGRO Topolyzer and T-CAT have been optimized to complete the range of
customized laser vision correction and to interact in perfect
synchronization with the ALLEGRETTO WAVE excimer laser systems. All
necessary treatment and patient parameters are transferred from the
ALLEGRO Topolyzer to the ALLEGRETTO WAVE laser systems via diskette,
mini CD or memory stick. The ALLEGRO T-CAT software in the laser´s
notebook then uses the data to calculate the appropriate correction
pattern. The physician can decide on the adaption of individual
parameters at any stage of the operation.

The integrated real keratometer and automatic measuring function allow a
precise centration of the image. Digital image transmission and
sub-pixel approximation combined with a comfortable working distance and
a measuring time amounting to only a fraction of a second ensure the
highest possible reproduceability and accuracy of the data recorded. All
data obtained here will be used by the ALLEGRO T-CAT software to define
the respective treatment parameters.

The ALLEGRO Topolyzer can also be used only as a diagnostic instrument
for regular topography measurements which are required in refractive
surgery.

A keratoconus detection software is already integrated and optional pupillometry software is also available.

The manifold screen views of the ALLEGRO Topolyzer include:

  • Overview display
  • Large color map
  • 3D cornea
  • Fourier Analysis
  • Zernike Analysis
  • Indices (keratoconus detection software)
  • Comparative examinations with difference map
  • Refractive display mode
  • Height data
  • Camera image

Applications for the ALLEGRO Topolyzer include:

  • Repair station for decentered / irregular ablations after previous refractive surgery
  • Enlarging small optical zones
  • Keratoconus screening
  • Treatment of high astigmatism
  • Treatment of other surgically induced corneal irregularities
